I believe 'Form D' actually contains the same info as a Train Order (Form 19) but is not called a "Train Order" per se; When I left AMTRAK in 1988, we did not have Form D; I believe Conrail started that procedure in 1987. 
I surmise that CR started doing this because Block Operators were paid 3 hours pay when radioing a Train Order... so, therefore operators (what were left) would be radioing a "Form D" instead.
